Eligibility Forms

To become eligible to compete as an SCC athlete, all applicants must complete the following forms:

• Student Athlete Form
• Student-Athlete Physical Examination Form

Before completing the forms, student-athletes will need to know their Shoreline Student Identification Number (SID) & Pin.

The Student Athlete Form will take approximately 20 minutes to complete. If you exit the form before it is completed, the information will NOT be saved. As the form is filled out, an option to auto-fill general information from the first page will be provided.

Please Note: If the student-athlete is under the age of 18, a parent or legal guardian must be present as they complete the form so the parent or legal guardian can sign where needed.
