How to apply to Phlebotomy
Certificate On-Hold
Due to lack of clinical placements, the phlebotomy certificate program is suspended until further notice.
Admission to this program is through an application process. Applicants need to meet the eligibility requirements and apply to the program as well as the College. Faculty and staff are available to assist with the process.
Applications are accepted once annually for all qualified applicants starting in both Spring and Fall quarters. Early or late applications will not be accepted. On the application, you may indicate if you wish to start during the Spring or Fall quarter.
Application fee: $35
Interested applicants must attend a mandatory Information Session.

Selection process
Applicants meeting program requirements and passing a criminal background check will
be entered into a lottery process. Applicants with lottery numbers 1-10 (this may
be less than 10) will be offered Spring/Summer program admission. The next 10 lottery
numbered applicants will be offered Fall/Winter program admission. The next 10 lottery
numbered applicants will be placed on a waitlist for these program times. Any applicant
not offered admission at this time may reapply for admission the following year.
Selected applicants will emailed a program admission offer and acceptance form. Individuals
on the waitlist will be notified of their status. Acceptance forms must be returned
by the deadline listed on the form. If this form is not returned by the deadline,
an alternate applicant from the waitlist will be offered admission. If space becomes
available for Spring, students admitted for Fall quarter will be contacted first to
fill those spots. This may open an opportunity for someone on the waitlist. If there
are an inadequate number of applicants to fill open slots, the application process
may be re-opened.

Applicants must pass a criminal history background check, show proof of a high school diploma or GED certificate, and submit all items in the application checklist
Prerequisites
ENGL 099 completion (unofficial transcript) or placement into ENGL&101

- Apply to Shoreline Community College to receive a student ID & PIN number.

Phlebotomy Program application checklist
Check off items as completed and submit with application.
- I attended a Phlebotomy Program information meeting on _________/___________/___________ (Required)
- Submit a non-refundable check or money order for $35 payable to Shoreline Community College ($20 covers the cost of application processing and $15 covers the cost of a Washington State Patrol background check.)
- Submit a completed Phlebotomy Program Application
- High School Diploma, GED, or college degree (copies)
- Provide transcripts (unofficial):
ENG 099, EAP 099 or
ACCUPLACER test results showing equivalent level of proficiency - Sign and return the “Technical Standards/Essential Skills Requirements” form
- Complete and sign a Washington State Patrol Criminal History form
- Submit two (2) Recommendation forms under separate cover or in a sealed and signed envelope.
- HIV/AIDS Education Certificate (7 hours) or transcript—NOTE: If you do not have HIV/AIDS training you will be enrolled in MLT 123 AIDS Education and required to complete the course.
Upon Acceptance
Upon acceptance, you will be required to:
- Provide proof of vaccination for the following:
- Hepatitis B Vaccination (3-shot series)
- TDap (within past 10 years)
- MMR (Measles, Mumps, Rubella) (2 shots)
- PPD: Two tests at least 1 week apart OR documentation of annual testing
- If student tests PPD-positive or had a BCG immunization, a chest x-ray report & Symptoms Checklist will be required
- Varicella and Influenza may be required by some facilities
- Provide copy of valid American Heart Association Healthcare Provider CPR card
- Provide proof of Health Insurance Coverage
